Advance Directive

Advance Directive

An Advance Health Care Directive (AHCD) are written instructions about your future medical care. They do not go into effect until you are no longer able to make decisions. Advanced directives have several functions:

  • Allow you to decide ahead of time what medical procedures you do or don’t want. This usually involved decisions about breathing machines, CPR, giving you food and water if you are unable to take them by mouth or the use of certain medications (antibiotics).
  • Helps your family make decisions.
  • Ensures your wishes are followed if they are different from your family’s wishes.

If you have questions, please talk to your Provider. To complete the Advance Directive forms, please ask your SCIHP Provider for a copy of the Advance Directive form or go to the California Attorney General website at http://ag.ca.gov/consumers/pdf/AHCDS1.pdf
