Indian Preference Form:
In order to receive Indian Preference, please download and complete the Indian Preference Form and return the form with your employment application to Human Resources.
Applications are accepted for existing vacancies only. Applications that are accepted by the Human Resources Department will be retained for a calendar year from the date of receipt. Appropriate testing may be given for skill requirements. In addition, you are required to submit a copy of your Driver’s License along with your application for employment, if needed.
Preference in filling vacancies is given to qualified Indian applicants in accordance with the Indian Preference Act (Title 2 U.S. Code, Section 472 and 473) and the Indian Self- Determination Act, Public Law 93-638, provided the applicant has submitted acceptable proof of Indian preference for employment. If you are claiming Indian Preference, the form must be returned along with your application.

NOTICE OF DRUG-FREE WORKPLACE ACT
In accordance with the Drug Free Workplace Act of 1988, SCIHP has adopted a Drug-Free and Alcohol-Free Workplace Policy and Procedure.
It is unlawful for any employee to manufacture, distribute, dispense, possess or use a controlled substance in the workplace. In addition, SCIHP must be notified within five (5) calendar days of any criminal drug statute conviction for a violation occurring in the workplace. Employees who are suspected of violating SCIHP's Drug-Free and Alcohol-Free Policy may be required to submit to an inspection of their person, vehicle or personal property. This may include medical testing for alcohol and/or drug use.
It is the responsibility of the SCIHP Board of Directors to ensure that the providers that we employ are licensed and eligible to practice.
SCIHP has a credentialing review process that verifies the identity, training, experience, and competency of an individual. Through assessment and validation of a provider’s qualifications to provide patient care services, the appropriate department director provides recommendations to the Credentialing Committee and the governing body regarding appointment and the scope of clinical privileges to be granted to the individual.
Appointment is determined on evaluation of the individual’s current license, training, experience, and current competence and ability to perform privileges requested.
